Age Spots: Why They Arrive in Patches, Not All Over

What people notice first is this: it is never everywhere at once. A flat brown mark on the back of one hand. A small cluster near a temple. A patch on one cheek and almost nothing on the other. If age spots were simply "getting older," they would show up evenly, like grey hair. They do not. They show up in a pattern, and once you see that pattern, the mirror makes a lot more sense.
Those flat brown marks have a name, solar lentigines, and despite the old nickname "liver spots," they have nothing to do with your liver (MedlinePlus, National Library of Medicine). What Age Spots Actually Are
Deep in your skin sits a small workforce of pigment-making cells, the melanocytes. Their job is to make melanin, the brown pigment that colours skin and, importantly, absorbs ultraviolet light. Melanin is protective. When sunlight lands on skin, melanocytes make more of it. That is what a tan is: your skin building a little shade for itself.

Sunlight rarely lands evenly, so the pigment builds up more in some places than others.
Do that for fifty years and the record begins to show. Age spots are the places where, over time, the most light landed and the most pigment built up. That is why they favour the backs of the hands, the forearms, the shoulders, the face, and a bald scalp, the places that see the most sun over the years (MedlinePlus, National Library of Medicine). They are common, too: by age fifty, most fair-skinned adults have at least a few. The patchiness is not random. It reflects where you rolled up a sleeve, drove with an arm on the windowsill, or walked the dog in the afternoon sun.
What Actually Slows Down Around Fifty
This is the part that surprises people, because the obvious guess is wrong. You might assume a spot is a cluster of pigment cells working overtime and making extra melanin. When researchers looked closely at age spots under the microscope, that was not quite what they found. In one careful study, age spots held about twice the melanin of nearby skin, yet the melanocytes were not more numerous and were not reading as more "switched on" than the skin around them (Choi et al., 2017; PMC).

So where does the extra pigment come from? Mostly from a slowdown in how skin clears old cells. Healthy skin turns over steadily. Cells are born at the bottom, rise toward the surface, and shed, carrying their pigment up and off with them. That turnover slows with age. The same study found the pigment was accumulating because it was not being cleared and moved along the way it used to (Choi et al., 2017; PMC). And the pigment cells themselves change with the years, drifting into a sluggish, less responsive state that tracks with age (Hughes et al., 2022; PMC).
That is what is really behind the idea that something "breaks" at fifty. Nothing snaps. No single switch flips. The rhythm of making, moving, and shedding pigment simply gets less even with time. In the patches that took the most sun, pigment arrives faster than the skin can clear it, and it stays behind.
Skin and Circulation
There is one more piece here, and it matters. Your skin is one of the most heavily supplied tissues you have, fed by a dense mesh of the tiniest blood vessels. That delivery network thins with age; the number and density of small vessels in aged skin drops (Bentov and Reed, 2015; PMC). Less delivery can mean slower repair and renewal.
To be clear and fair about it: circulation does not cause age spots, and supporting it will not erase one. Sun did that, over years. But your skin's resilience, its capacity to renew, and its ability to recover do depend on that microcirculation. What Actually Helps
None of the below is a fix, a promise, or a product. Age spots are usually harmless and don't need treating at all (MedlinePlus, National Library of Medicine). What follows is simply what the evidence supports if you'd rather your skin ages a touch more evenly from here.
- Protect from here forward. This is the one with real trial evidence behind it. In a randomised trial, adults who used sunscreen daily showed 24% less measurable skin aging over four and a half years than those who used it only now and then (Hughes et al., 2013; PubMed). You cannot undo the last thirty summers, but you can stop adding to the map. Daily broad-spectrum protection, a hat, and shade in the strong hours do most of the work.
- Studied topicals, with a professional. A number of ingredients are studied for evening skin tone, retinoids, vitamin C, niacinamide, and gentle exfoliating acids that help nudge slow turnover along. Results are modest and slow, and the right choice depends on your skin. This is a conversation for a dermatologist, not a shopping cart.
- Support the skin underneath. The unglamorous, repeatable basics, steady movement, a plate built around plants and fish, real hydration, and protected sleep, are what keep skin's delivery system and renewal working. One caution worth stating plainly: age spots are flat and even-coloured. If a mark changes, grows, darkens unevenly, itches, or bleeds, that is not cosmetic, and it's a reason to see a doctor promptly (MedlinePlus, National Library of Medicine).
